Chronic Disease Programme

Fitness and health tools including a glucose meter, skipping rope, apple, and running shoes.

Chronic Disease Programme (CDM)

The 6 monthly Chronic Disease Management Programme is for patients who have a medical card, GP Visit card or a Health Amendment Act card and have a specified chronic disease: asthma, COPD, type 2 diabetes or heart disease.

Prevention Programme (PP)

The annual Prevention Programe is for people who have a medical card, GP Visit card or a Health Amendment Act card and have a diagnosis of hypertension or who are at high risk of cardiovascular disease or diabetes, and all adults aged 18+ diagnosed with gestational diabetes or pre-eclampsia since January 1, 2023.

Benefits

This programme is designed to support you and your GP in managing your chronic condition effectively and reducing the risk of hospital admission. Benefits of the programme include:

  • A personalised care plan tailored to your individual needs, preferences, and goals
  • Ongoing review of your treatment plan and medications by your GP
  • Support and guidance to help you manage your condition confidently
  • Access to structured education and self-management resources
  • Early identification of any new health conditions
  • Early detection and management of complications related to your condition
  • Convenient care provided within your local community, close to home

How It Works

When you prescription expires please contact the office to book in for Visit 1 with Nurse Jill.

Visit 1

At this appointment you will undergo the following:

  • Weight, height and BMI measurement
  • Waist circumference
  • Heart rate & rhythm check
  • Blood pressure check (if elevated you will be booked in for a 24 hour BP monitor)
  • Blood tests relevant to your condition
  • Electrocardiograph (ECG)
  • Lifestyle review

Visit 2

One week later you will follow up with your usual doctor or nurse prescriber. At this appointment we will review any findings from Visit 1, including your blood tests. We will then issue you with a 6 or 12 monthly prescription to cover you until your next review.
